3. State whether you need physical space, and what kind
4. Ask for licence, facility, establishment card and visa costs itemised separately
5. Ask what the renewal figure will be, not just the first year

Quotes that come back dramatically cheaper than the rest have almost always dropped the visa costs or assumed a facility that does not support your headcount. The gap appears after the licence is issued, when changing your mind is expensive.
